The Duke Raleigh Department of Pharmacy, led by the Director of Pharmacy, is comprised of on-site pharmacies as well as off-site pharmacy facilities. The pharmacy divisions located within the hospital include the inpatient central pharmacy, an oncology pharmacy, an operating room pharmacy, and an outpatient pharmacy (Plaza Pharmacy). Beyond the hospital, there is an outpatient surgery center and cancer care center pharmacy (Macon Pond Pharmacy). The pharmacy residents have exposure to all aspects of the pharmacy department and access to all hospital and health-system resources.

Pharmacists at Duke Raleigh are integrated members of the interdisciplinary medical team, working collaboratively to provide pharmacotherapy recommendations to achieve optimal patient outcomes. Clinical pharmacy services are provided via an integrated practice model of centralized and decentralized pharmacist roles. Under several Pharmacy & Therapeutic Committee-approved policies and practice agreements, pharmacists independently manage total parenteral nutrition, therapeutic drug monitoring of select medications (e.g. warfarin, vancomycin), intravenous to enteral conversion, renal dosing adjustment, and antimicrobial stewardship services. Pharmacists also participate in research studies through a partnership with Duke’s Investigational Drug Study team.
The Oncology Pharmacy provides on-site pharmacy services for inpatient chemotherapy and the Duke Raleigh Cancer Center. The oncology clinical pharmacists provide high-quality clinical care for approximately 70-80 patients a day with nearly all types of cancer including solid tumors and both benign and malignant hematologic cancers. Treatment plan reviews are completed for patients on standards of care as well as those enrolled in clinical trials.
Oncologic clinical pharmacy services are provided via collaboration across the pharmacy department including the oncology clinical pharmacists, the inpatient decentralized pharmacist, and the hematology/oncology clinical pharmacist. The hematology/oncology clinical pharmacist is primarily located in the outpatient oncology clinic as part of the interdisciplinary team coordinating and monitoring care for Duke Raleigh cancer patients.


The Operating Room Pharmacy serves the pre-, intra-, and postoperative patients by providing clinical and technical pharmacy services to enhance surgery outcomes. They support this goal by collaborating with surgeons, anesthesiologists, nurse anesthetist, nurses, and other ancillary staff to deliver patient centered care by utilizing medications in a safe and cost-effective manner. The Operating Room Pharmacy team oversees controlled substance dispensing and waste in the perioperative areas. Under several Pharmacy and Therapeutic (P&T) Committee-approved policies and practice agreements, the pharmacists assist with renal dose adjustments and antibiotic allergy assessment to increase compliance with the Surgical Care Improvement Project (SCIP) measures.
Duke Raleigh Hospital Plaza Pharmacy is a full-service outpatient pharmacy open Monday-Friday 9:00-16:30. Plaza Pharmacy services are available to patients who would like to fill prescriptions before leaving the hospital campus. In addition to dispensing new prescriptions, the Plaza Pharmacy also provides the following services:
- Medication education to ensure patients understand indication, dosing instructions, side effects, storage and disposal
- Assistance for patients who need to obtain manufacturer coupons to reduce or eliminate co-pay
- Discharge medication review for patients at high risk for hospital readmission

For patients admitted to Duke Raleigh who are anticipated to have difficulty in obtaining prescriptions (i.e. no friends/family available to help pick up prescriptions), Plaza Pharmacy offers a “Meds to Beds” program. This program entails bedside prescription delivery by a pharmacist who is able to review new medications with the patient and address any questions/concerns.
In addition to serving patients, Plaza Pharmacy is also available to Duke employees for both prescription and over-the-counter medication needs.
